The New Zealand MidCentral District Health Board, (DHB) serves a population of 162,500, the majority of whom live in Palmerston North City. Approximately 16.5 per cent of the population is aged over 65 years and Maori make up over 17 per cent of the DHB’s population. The DHB stretches across the lower North Island from the west to the east coast of New Zealand.
The Elder Health and Rehabilitation cluster is responsible for older persons health, rehabilitation and wellbeing at a district level and In close consultation with the Chief Executive and in partnership with the Clinical Executive the purpose of the role is to demonstrate effective operational leadership and decision making, whilst ensuring a person and community focus, that it is integrated across primary, community and specialist care providers, as well as with and between other clusters. You will lead the process of defining, designing and developing the Elder Health and Rehabilitation cluster using a co-design process with the outcome of improving the individuals health and wellbeing, experience of care, and the health of populations
